Shipments to the United States or overseas require specific documentation. Improperly completed or missing documents result in delivery delays. In order to help with this process, Midland Courier has prepared a list of forms required for different types of shipments, as well as some helpful tips for completing them. Standard Custom Document / Commercial Invoice
This form must be completed for all International Shipments.

NAFTA Certificate of Origin
For shipments containing commercial goods that qualify for NAFTA origin. Rules of origin are complex and may be determined using a number of different factors. Please contact your custom broker to determine if your shipment requires NAFTA documentation.
Export Declaration B13A
For shipments with a declared value of $2,000 or over
FCC Form 740
For shipments containing computer / radio related items
FDA Radiation Form FD2877
For shipments of items that may result in radiation exposure
Single Country Declaration (Fabrics)
For shipments including any fabric materials
Video Film Declaration
For shipments containing any video or motion picture film
